Survey Guidelines

Privacy:

FETCH will use the information collected to improve our paper and plan future issues of FETCH. We may publish the collected survey results, but we will not use any personally identifiable information without your knowledge and permission. See the FETCH Privacy Policy for more information.

Prizes:

FETCH will award prizes to selected individuals answering the complete survey. Survey takers must provide us with a current email address or phone number where we can reach you, so that we may contact you and know where to send the prize.
